-BUILDING USAGE INFORMATION SHEET-
Requests for any use of any building facilities shall be made by submitting the attached Building Use Request Form. The Building Proprietor will review all requests. After review, the applicant will be contacted to complete the process. Once the application is reviewed and approved, the event will be added to the Building Use Calendar.
A deposit of $100.00 must accompany all requests for building use. The applicant must be the host of the event and remain on St. Mark’s Building property for the duration of the event. Upon approval, a Building Usage fee must be paid using the schedule below:

FACILITY USE CONDITIONS AND RULES:
Historic St. Mark’s reserves the right to reject any request for the use of the facilities. Moreover, HSM reserves the right to cancel the usage of facilities for any reason at any time without prior notice. The following rules and conditions apply to the use of the HSM facilities:
CONDITIONS:
1. Requests may be made between three (3) months and 14 days prior to the event date. Should the party requesting the use of the facilities need to cancel, notice must be given as soon as possible. Failure to notify HSM proprietor of a cancellation within 7 days of the scheduled use will result in the forfeiture of all fees and deposits.
2. Anyone using the facilities agrees to indemnify, save, and hold harmless HSM of and from any claims, actions, causes of action, demands, rights, damages, costs, and expenses, including reasonable attorney’s fees, arising out of any incident, accident, or event that any person using the facilities causes any harm to any individual or to any HSM.
3. Organized non-profit groups must provide HSM with proof of insurance and add HSM as an additional insured as it pertains to the group’s insurance policy. Groups must carry a minimum limit of insurance of at least $500,000 per claim and $500,000 per occurrence. This proof of insurance (certificate of insurance) must be submitted to the church office at least fourteen days prior to the requested date of facilities use.
4. Anyone using the facilities must leave them as they were found. Any damage to the facilities (including discharging of fire extinguishing equipment) will be the responsibility of the person(s) requesting the use of the facility. Repair or replacement of the damaged property will be at the sole discretion of HSM. The deposit submitted with the request form will be held pending inspection and inventory and will be returned within 30 days after the event. The user assumes liability for injuries to persons attending the event and for damages or loss of user’s property.
5. Deposits and Fees: A deposit of $100.00 must be submitted along with each Building Use Request Form. The fee(s) must be paid at least 14 days prior to the requested date for use of the facilities.
6. All trash and refuse must be bagged, tied, and kept in the trash container to be removed by the custodian.
7. The building must be vacated by the end time listed on the request form. All events must be conclude by 11:00 p.m.
8. The member hosting the event must be on the premises the entire time requested.
9. All items associated with the event must be removed immediately following the event. HSM assumes no responsibility or liability for items or property brought onto or left on the property by any party.l HSM reserves the right to dispose of any item or property that has not been removed 14 days after the event.
FACILITY RULES:
1. The following are not allowed on HSM property at any time: weapons, tobacco products, controlled substances/drugs. Alcoholic beverages should be used with discretion and not distract from a Christian atmosphere.
2. No animals are permitted within the facility except those assisting a person with a disability or unless HSM has expressly granted permission in the event a special program is scheduled.
3. Anyone using the facilities must do so in a responsible manner, treating the facility with dignity and respect. Those using the facility will not exhibit disruptive behavior or use inappropriate language..
4. Facility users must provide a minimum ration or 1:8 of adult chaperones for any event involving minors and NO LESS THAN two adult chaperones (over the age of 18) present at any time. When children are in attendance, they must be under control of their parents or adult chaperones at all times and are not permitted to roam freely on HSM property.
5. All users are required to remain in the room(s) specified and approved on their Building Use Request Form and are not to enter any other areas of the building except the restrooms. Moreover, they are also required to enter and exit only through the designated entrance(s) and exit(s).
6. Food and beverages may not be consumed beyond the Fellowship Hall and/or kitchen without prior approval by the Building Proprietor.
7. No decorations, posters, photographs, or other items may be mounted or affixed to any walls, ceilings, or woodwork using any technique that would mar the wall surface. Easels are available upon request. Confetti and glitter are not permitted.
8. No HSM property shall be removed from the facility unless advance written approval from the HSM proprietor is granted.
9. The use of the facility must not interfere with other building usage activities.
10.The facility will not be used for any business purposes or fund-raising other than those authorized by the HSM proprietor. To avoid any scheduling, security or maintenance conflicts, all building usage must be placed on the calendar via the HSM Proprietor. No admission or related fees will be charged by the entity requesting facility use unless approved by the building proprietor.
11.Supplies (kitchen paper products, coffee, craft supplies, etc.) belonging to HSM, with the exception of Bunn coffee filters, are not to be used.
12.The use of candles is prohibited. The use of battery operated candles is recommended.
13. If furniture or equipment is to be moved, it must be done with permission of the Proprietor to maintain the quality of the building’s furniture and equipment and to prevent damage and/or injury.
KITCHEN USAGE SPECIFICS:
When the Fellowship Hall is rented for an event, this includes the use of the refrigerator, freezer, coffee maker,(including (lg. filters), the gas stove, microwave, and the sinks. Note: The stove top is to be used for warming only….No frying.
No paper products found in the pantry are to be used for any non-HSM sanctioned events. Please bring your own disposable items. If you use any of our dishes for any reason, please wash, dry and return them to their designated spot.
Willful violation of any rules or conditions could lead to removal from HSM property, forfeiture of deposit and/or loss of eligibility to use the facility
